Equity-oriented palliative care teams
We collaborate with mobile palliative care teams and practitioners across Canada in research, service development, implementation, and evaluation. These teams work to provide and advocate for better access to palliative care for their clients living homeless and vulnerably housed, and impacted by stigma, racism, and isolation.
In Victoria, the Palliative Outreach Resource Team (PORT) began in 2011 with service providers working off the side of their desk to improve care. In 2015, we began research to highlight the experiences of people at end-of-life in contexts of inequity, and conduct collaborative research to improve access to care. Today, PORT is a sustainable program, a product of collaboration between University of Victoria, Island Health, and Victoria Cool Aid Society. Our collaborative acts as a bridge between people with serious illness and their caregivers, palliative care, and other health and social support systems.
