Advance Care Planning (ACP) has emerged in our Action Team as a focus to ensure that people’s values, wishes, and bodies are respected at end-of-life and after death. To date, we have done several workshops with inner city organizations and developed an ACP template that is being widely used. In April, our work was featured in the Globe & Mail.

Justine Hunter writes, “Now, a group of front-line workers and palliative-care experts have joined forces in Victoria to help those most at risk of an untimely death to make effective end-of-life plans. They are adapting the principles of palliative care – from pain management to emotional support for those who are dying – to the needs of vulnerable and marginalized populations.”
